Pre-Registration Immunization Requirements
All students must provide immunization records prior to registration at The University of Montana. Both a “Pre-registration Immunization Requirements form” and a legible copy of immunization records or a statement from your doctor are required.
Immunization records are usually required only once prior to your first registration at UM, but it is always a good
idea to keep a copy of your records on file. UM-Missoula and UM-Western require a separate submission of
immunization records.
Steps to meet this requirement
Get the form:
Fill out part one:
- Fill out the top portion of the form. Note: You must provide your email address. If the Curry Health Center needs additional information, this is how they will contact you.
Fill out part two:
- On the left-hand side of the second half, you may do one of two things:
- You may have your doctor complete this section and sign it, or
- You may attach a current legible copy of your immunization records.
Fill out part three:
- Fill out the Tuberculosis History information. Depending on how you respond to the question in this section, you may be required to have a TB test.
Sign the form at the bottom, right. Once completed, the form or the form plus attached immunization records can be faxed to Curry Health Center at 406-243-6955.

* If you have been admitted to the LME program, but have not taken classes at UM-Missoula for the past two years,
you will need to resubmit immunization records as part of the re-enrollment process.
