New students must provide a completed pre-registration immunization requirement form that shows a physician-validated immunization record for measles, rubella, diphtheria, tetanus, polio and skin testing for tuberculosis. If you were born after December 31, 1956, you must submit a certificate of immunization against Rubella and measles (Rubeola). You will not be allowed to register until the UM's Curry Health Center has received proof of your immunization.
