I-94 Form "Arrival & Departure Record"

Visa: Generally, but not limited to F-1/J-1/TN

TN Visa Holders: Photo ID and copy of visa

F-1 Visa Holders: I-20 Form "Certificate of Eligibility for Nonimmigrant Student Status"

F-1 Visa Holders: Copy of visa and Letter of Work Authorization

J-1 Visa Holders: DS-2019 "Certificate of Eligibility for Exchange Visitor (J-1) Status"

J-1 Visa Holders: Copy of visa and Letter of Work Authorization

Local Residence Address - (not a Department/PO Box/32 Campus Drive Box address)
