Administrators, Contract Professionals

The following forms must be completed before an employee's first day: 
  • MUS Contract or Letter of Appointment 
  • Personal Information Form (completed by employee)
  • Background Check (completed by employee)

Compile the following documents and upload in U-Approve under Supporting Documentation:

  • Letter of Offer (MUS Contract) - To be reviewed by HRS Recruitment Manager prior to giving to candidate.
  • Transcripts
  • Vitae or resume (if not hired through SilkRoad Recruiting)

The following forms must be completed on or before an employee's first day and submitted to Human Resource Services, Lommasson 252.  These forms are not completed in the U-Approve process.
