The following forms are used to document the internship experience. Students and faculty internship advisors may review these forms at anytime in the GRIZ eRecruiting.
We are available to assist employers and interns encountering difficulties during an internship. Internship Services staff conducts a limited number of site visits each year. At your request, we may also be able to facilitate conflict resolution with your intern. Please feel free to call us if we can help in any way.

| Intern Request Form -- Completed by the employer to request an intern. This form can be used by those employers who prefer to have Internship Services post the internship opportunity on Griz eRecruiting. However, the employer may create their own Griz eRecruiting account and post/edit/manage their own internship opportunities rather than completing this online form. |

Online Learning Agreement -- Completed by the intern after getting appropriate information from the intern's faculty internship adviser. This form outlines responsibilities and learning objectives for the internship. It should be completed by the third week of each semester. The student will need a GRIZ eRecruiting account to complete the Online Learning Agreement. |

| Supervisor's Evaluation of the Intern -- Completed by the intern's supervisor. This form allows the employer to evaluate the intern's performance and progress. It is emailed to and completed by the supervisor during the last two weeks of the semester. |

| Student File Registration - Completed by the student. Information on this form will be used to help match skills, interests, and location preferences with internship opportunities. Students who arrange their own internship do not need to complete this form. |

| Student's Evaluation of the Internship Experience -- Completed by the intern. This form asks about the internship experience, and welcomes suggestions for future interns or the employer. It is emailed to and completed by the student during the last two weeks of the semester. |
