College of Technology Graduation Application

The University of Montana Graduation Application 09-10

The University of Montana Graduation Application prior to 09-10  
This application is for the first UM degree only. Second degrees or second majors must use another form available in the Graduation Office - Lommasson 201 or the Griz Central Registration Counter.  There is a $30 graduation fee for the first degree, which is to be paid at the Griz Central Cashier Counter once the application has been approved and signed by your major department chair.  You must have your application in hand when paying the application fee.

Course Addition Form
This form is used by academic departments to add a course for a particular term.

Course Change Form
This form is used to change information on an existing course. For example, times, days, instructor, grade option, etc.

Confidentiality
This form is for requests to withhold or include directory information. You may choose to have directory information withheld from third parties, have extra confidentiality noted with your addresses as well as directory information withheld or have your directory information included in The University of Montana directory.

Readmission Application for Undergraduate Students
Students previously enrolled at The University of Montana not been enrollment for 24 months or more must submit an application for readmission. This link provides the required form. For more information, please contact Linda Jesse or click on readmission information.

Registration Override
This form is used within the 1st through 15th instructional days of the current semester in progress. This form is used for restrictions on courses such as: Major Restrictions, Class Restrictions, Closed Courses, Consent of Instructors and Consent of Honors. Do not use this form after the 15th instructional day of the semester.  Please contact Matt Filer for more information.

Reinstatement Forms for Students on Academic Suspension
