Application for Associate of Arts

Application for Certificate

The University of Montana Application for Bachelor Degree
This application is for the first UM degree only!  Second degrees or second majors must use another form available in the Graduation Office - Lommasson 201 or the Griz Central Registration Counter.  There is a $25 graduation fee which is to be paid at Griz Central Cashier Counters once the application has been approved and signed by your major department chair.  You must have your application in hand when paying the application fee.

Course Addition
This form is used by academic departments to add a course for a particular term.

Course Change
This form is used to change information on an existing course. For example, times, days, instructor, grade option, etc.

Confidentiality
This form is for requests to withhold or include directory information. You may choose to have directory information withheld from third parties, have extra confidentiality noted with your addresses as well as directory information withheld or have your directory information included in The University of Montana directory.

Distance Learning Change of Status Form

Drop/Add
ONLY USE THIS FORM FOR THE CURRENT TERM IN PROGRESS.
This form is used to drop/add courses, change grade options, variable credits, section changes within the 16th through the 30th instructional days. Please contact Kerry Herndon for more information.

Drop/Add (Law School)
This form is used add courses and change sections for the School of Law. Drop/Add deadlines available in Registration packet at the Law School.

Petition to Change Registration after 30th Instructional Day
This form is used to petition changes after the 30th instructional day of a particular semester.

Readmission Application for Undergraduate Students
Students previously enrolled at The University of Montana - Missoula who have interrupted their enrollment for 24 months or more must submit an application for readmission. This link provides the required form. For more information, please contact Linda Jesse.

Registration Override
This form is used within the 1st through 15th instructional days of the current semester in progress. This form is used for restrictions on courses such as: Major Restrictions, Class Restrictions, Closed Courses, Consent of Instructors and Consent of Honors. Do not use this form after the 15th instructional day of the semester. Please contact Kerry Herndon.

Reinstatement Forms for Students on Academic Suspension
Following suspension, students will generally not be considered for reinstatement until at least one semester has passed. For information regarding reinstatement, please see Reistatement to the University of Montana.

Summer Override Drop/Add
This form is used for restrictions on courses such as: Major Restrictions, Class Restrictions, Closed Courses, Consent of Instructors and Consent of Honors. Please use this form after courses are not available via the web for the semester. Please contact Kerry Herndon.

Transcript Request Form
Transcripts are ordered through the Registrar's Office. This link provides the required information and form you will need to place an order. For more information, please see "Transcripts," or contact Kathy Brager.

Withdrawal - Current Semester
This form is not yet available online. We apologize for any inconvenience.

Withdrawal - Retroactive
This is a petition for withdrawing after the 14th week of attendance. For further information, please contact Linda Jesse.
