Documentation Requirements

The Office for Disability Equity asks students who request reasonable accommodations to describe their disability, their past use of accommodations or the disability's likely impact on their educational experiences.

Types of helpful documentation supportive of such requests include:

  • medical records
  • psycho-educational testing
  • school records (e.g., Individualized Education Plan or 504 Plan)

We suggest that the documentation includes the following information: 

  • Diagnostic statement identifying the disability with the date of diagnosis.
  • Description of the current impact and functional limitations caused by the student’s disability.
  • Severity of the disability: mild, moderate, or severe.
  • Approximate duration and prognosis of the disability.
  • History of clinical contact with student including the date of the last clinical contact.
  • Statement of how treatment such as medication impacts the student's functioning in an academic, workplace, and/or residential setting.
  • Recommendations for accommodations in order to equalize the student's educational opportunities at the University of Montana. (e.g., test taking, note taking, comprehension, and/or class attendance).
  • Credentials and contact information of the evaluator(s) including the professional's name, title, certification/license, mailing address, telephone number, and signature.
