First report of injury forms:

First Report paper versionFirst Report electronic version

First of all, the University of Montana (UM), as part of the Montana University System (MUS), want all UM employees to arrive safely at work, to be able to perform one’s respective job duties in asafe manner and in a safe environment, and then to be able to return home safely.

However, sometimes there occurs an event such that the employee is injured on the job, or after several months there’s an increase in pain in a muscle or joint that may be linked to some repetitive task performed at work over time.

If this happened to you would you know what to do?

  1. Know that you are not alone.  There is a step by step quick instruction sheet that is designed to quickly assist you and your supervisor to take prompt action.

  2. After reading instruction sheet click on above picture (paper or electronic) to access the First Report of Injury form.  We recommend you try the electronic option but either form needs to be signed by the employee and reviewed by supervisor before processing can begin.
