This form must be completed in order for UM students, faculty/staff/students from UM affiliate campuses (UM Western, MT Tech, and Helena College), and external investigators engaged in research who are not UM faculty or staff to gain access to the Cayuse system. When received by the UM IRB office, this form will be processed as soon as possible. Please allow up to 48 hours for your credentials to become active. UM faculty and staff have access to Cayuse and do not have to complete this form.
