Dental Insurance Billing
The student Blue Cross Blue Shield insurance DOES NOT COVER for dental charges.
If you have outside dental insurance, an insurance claim form will need to be filled out by the student each time they receive services at the dental clinic.
A standardized claim form can be obtained at the Dental Clinics’ front window, otherwise please bring a specific form if required by your specific insurance company.
Please tell the Dental Clinic receptionist that you have outside insurance when you check in for your appointment. The receptionist will submit the claim form with the charges after services are rendered. Ultimately, all charges are the responsibility of the student.
