In order for us to pay you prize money you must submit a completed University of Montana format W-9 for each team member you want to receive money directly from. Note that every team receives at least $250 in prize money. Check instructions on how to fill the form out correctly on our website (it’s complicated and you could get charged $100 if it’s submitted incorrectly!). Forms are due to Paul.Gladen@mso.umt.edu by no later than March 19, 2020.

Other important instructions to note as you fill out the form:
- Use an address that will be valid after the end of the semester. We find that a high % of students give us dorm or apartment addresses and then do not receive their check because they move. This creates an inordinate amount of additional work for staff that can be avoided by instructing the students to give a permanent address.
- If you provide a Social Security Number, the check will be issued in your name.
- If you provide an Employer Identification Number, the check will be issued to the Legal Name or Trade Name.
- Please make sure to sign and date the form, and print your name, title and phone number at the bottom.
- If only one person from your team is receiving the award money then only that person needs to fill out this form. If multiple people are receiving the award money all team members must fill out this form.
YOU WILL BE CHARGED A $100 PROCESSING FEE IF YOU HAVE TO CORRECT THIS PAPERWORK or we have to resend a check that could not be delivered to the address you provided.
