Name Change Information
Employees can update their name in University systems through HRS. There are three types of name changes available depending on your situation. Review the options below and bring the required documentation to HRS to complete your request.

Process

Legal Name Change
Use this option if you have legally changed your name through a court order or other legal process such as marriage or divorce.
Required documentation:
- Completed Personal Information form (Name, Previous Name, and Name on Social Security Card fields of Section 1, plus signature at the bottom of Page 1)
- Present Government-issued photo ID for identity verification
- Provide document(s) of name change; court order, marriage certificate, etc
- Social Security card, to verify the name used for W-2 purposes
Full Chosen/Preferred Name Change
Use this option if you wish to use a chosen first and last name that differs from your legal name across University systems.
Required documentation:
- Completed Personal Information form (Name, Previous Name, and Name on Social Security Card fields of Section 1, plus signature at the bottom of Page 1)
- Social Security card, to verify the name used for W-2 purposes
Chosen First Name Change
Use this option if you wish to use a chosen first name only, while keeping your legal last name.
Required documentation:
- Completed Personal Information form (Name and Previous Name fields of Section 1, plus signature at the bottom of Page 1)
- No additional documentation required

Download and complete the Personal Information form. Fill in the fields that apply to your name change type as described above. Make sure to sign at the bottom of Page 1 before bringing the form to HRS.

Bring your completed form and all required documentation in person to HRS in the Emma Lommasson Center, Room 229. Name changes cannot be processed without the required documentation.
Things to Know Before You Submit
- All former and chosen names remain in system history
- A legal or full chosen name change will be fully reflected in the online campus directory. A chosen first name change will only be reflected in your email address
- Your email address will change following a name update
- Access to other campus systems may be affected. Check with individual system owners to confirm whether any additional steps are needed to avoid service interruption
- If you are also a student, check with the Registrar's Office to determine whether additional records or forms need to be updated in student systems such as Moodle
- Notify colleagues and contacts of your new email address after the change is processed

Yes. Name changes require in-person submission with original documentation. HRS cannot process name changes by email or mail.

It depends on the type of change. A full chosen name change is reflected in the online campus directory. A chosen first name change is reflected only in your email address. Other systems may vary. Contact individual system owners to confirm how your name will appear.

Yes. A name change processed through HRS updates your employee records. If you are also enrolled as a student, contact the Registrar's Office to determine whether your student records and systems such as Moodle also need to be updated.
