This form does not replace emergency reporting procedures. If there is an immediate threat to safety, a medical emergency, or suspected criminal activity, call 911 first.
If you suspect child abuse or neglect, immediately contact Montana Child and Family Services at 1-866-820-5437. After making the report, notify YSCO as soon as possible and no later than 24 hours.
Once appropriate medical care, first aid, or emergency response measures have been provided, submit this form within 72 hours of the incident to report significant injuries, illnesses, behavioral concerns, safety incidents, or other matters involving youth program participants. Examples include:
- Illnesses or injuries requiring more than basic first aid
- Mental health or wellness concerns
- Behavioral incidents or policy violations
- Bullying, harassment, or discrimination concerns
- Missing participant situations
- Parent/guardian complaints
- Situations requiring staff, medical, law enforcement, or administrative intervention
This form is NOT intended for routine supervision matters, including coaching, redirection, reminders about program expectations, or minor incidents that are promptly resolved and do not require further action or follow-up.
