Youth Program Registration
All Tier I, II, and III Youth Programs must register with the Youth Safety & Compliance Office (YSCO) and comply with all registration requirements listed below. Programs designated as exempt are not required to register.
Registration must be submitted at least 45 calendar days before the program start date of a Tier II or Tier III Youth Program, and at least 30 calendar days before the start date of a Tier I Youth Program. Late submissions may still be reviewed on a case-by-case basis.

Registration Form Requirements
The Tier I Registration Form AND the Tier II & III Registration Form will require the following information:
- Name and contact information for the designated Primary Point of Contact
- Program name, dates, and location
- Length of program
- Program type (residential or non-residential).
- Anticipated number of youth participants and age range
- Anticipated number of Program Staff members (Tier II and Tier III programs must provide a complete staff roster at least 14 days before the program start date.)
- Anticipated number of staff serving as Day Camp Counselors or Residential Staff
For Tier I Youth Programs, the Primary Point of Contact must also acknowledge the following:
- Notification confirming that your program will maintain youth protection policies, background check requirements, and supervision standards consistent with or equivalent to those required by the University of Montana.
For Tier II and III Youth Programs, the Primary Point of Contact must also acknowledge the following:

Staff Roster & Background Checks |
Provide YSCO a staff roster that meets required supervision ratios. Indicate whether each staff member is 18 years of age or older and their affiliation with the University of Montana. The Primary Point of Contact must also either certify that all required background checks have been completed and cleared by UM Human Resources or UMPD, or provide YSCO with the information necessary to facilitate required background checks, including each applicable staff member's first and last name and email address. |

| Participant Consent Forms By the program start date |
A Consent to Participate form has been or will be completed and collected from all participant families before participants engage in program activities. Required items include the following:
These forms must be retained (electronically or in paper form) in accordance with the MUS General Record Retention Schedule (Current Year + 6 years) |

Final Program Details |
Participant information, including each participant's full name and date of birth will be submitted to YSCO. |
Failure to meet these requirements or comply with applicable policies and procedures may result in the program being denied approval to operate or continue operating on campus.
