Lump Sum Bonus Form

A lump sum bonus is utilized to reward an employee who has demonstrated exemplary service or has made a significant contribution to the University.  These awards are issued in amounts of $100, $200, $350, $500, $750, $1,000, or $2,000.  Employees must meet the criteria outlined on the lump sum bonus request form in order to be eligible for this award.
