Form Builder

The Form Builder block allows you to create a simple email form that a visitor to your site can fill in with information you request on the form. That information is then emailed to an email address that you designate.

Please note:

  • If your site has never used forms before, permissions and access will need to be set up. Please email webhelp@mso.umt.edu for assistance.
  • You are not able to attach more than one form to a page.
  • The Payment function of the Form Builder block is no longer supported for new forms. Existing forms will continue to work for now. Please see our page on the Pay Now Button for the new payment option.

Create a Form Builder block

  1. Create the block.
    1. Click Add Content in the Top Menu Bar.
    2. Select Blocks > Row Blocks > Form Builder.
  2. Form Builder.
    1. Email Form To - Select the email address where you want to receive the form data.
    2. From Email Address Field - A confirmation email to the form creator will be sent from an email address entered into this field. The value entered into this field needs to be identical to the value entered into the form element created below. For example: 'Email Address.'
    3. Subject Field - A confirmation email to the form creator will be sent with a subject line entered into this field. The value entered into this field needs to be identical to the value entered into the form element created below. For example: 'Subject Line.'
    4. Thanks Text - The user will see this text after the form is submitted.
    5. Thanks Redirect Page or Thanks External Redirect - The user will be redirected to this page after the form is submitted.
  3. Form Element - Select the type of info that you want the user to enter here. To add a new form element, click the green + symbol at the top right.
    1. Type - Select the type of info you'd like in the dropdown, i.e., text, checkbox etc. 
    2. If you select checkbox, enter in the list of options the user can select in Dropdown/Checkbox Options.
    3. Advanced Options - Enter text here that will help the user understand what information you are asking for.
  4. Advanced Options 
    1. Unique Form ID - leave this set to the default.
    2. Secure Data - This checkbox is currently not working. It should redirect anyone who enters in the form URL as 'http' to the 'https' version of the form. This issue has been flagged for resolution.
    3. Display Form Elements Inline - Setting this to 'Yes' will display fields next to each other horizontally where space permits.
    4. Require Authentication - Setting this to 'Yes' will require a user to sign in with their Net ID in order to fill out the form.
    5. Show Clear Form Button - Setting this to 'Yes' will display a button that allows the user to clear all of their entries.
    6. Skip Rate Limiter - Setting this to 'Yes' will institute a protocol that will limit the amount of submissions to the form coming from the same IP address. This will help stop a spammer from using a program to automatically fill in a large number of submissions to your form.
      1. Please note that it will not stop all SPAM submissions, but will significantly limit them.
      2. Also note that this may interfere with a legitimate submission if it is completed within 60 seconds of the previous submission.
    7. The Payment function of the Form Builder block is no longer supported for new forms, but existing ones will continue to work. Please see our page on the Pay Now Button for the new payment option.
  5. Submit.
    1. When you have added all the content, click the blue Preview Draft button in the top right of the editing window. 
    2. Then click the blue Submit button in the top right of the editing window.
  6. Attach the block to your page.
    1. Select or create the Row you want to attach the Block to.
    2. Set the Row Content Type to Row Block.
    3. Click the Choose Block button in the Block field and navigate to and select the Block you just created.
    4. Click the blue Choose button in the top right.
