New Student Health Checklist

Welcome to UM! Take a few minutes to complete these important health-related steps to help avoid registration holds, appointment delays, or other surprises later on.

 Submit your immunization records (part of the pre-registration process in Griz Portal)

Immunization Requirements

The University of Montana requires all students to complete the following immunization requirements.

Incoming students: Most students complete these requirements through the Admitted Student Checklist during registration. 

Graduate Students: Navigate to the 'Graduate Students' page for more details.

Deadline: Immunization records must be received by the 15th class day of the semester to avoid disruption to your academic status.

Requirement 1: Measles, Mumps, and Rubella (MMR)

Students must provide official documentation showing two doses of MMR vaccine.

MMR Requirements

  • Both doses must be given after 12 months of age
  • There must be at least 28 days between doses
  • Vaccines may be given as a combined MMR vaccine or as individual measles, mumps, and rubella vaccines
  • Any dose given before 1968 is not considered valid
  • The second dose must have been administered after 1980

Documentation Requirements

Official records must include:

  • Student name
  • Date of birth
  • Vaccination dates

Alternative Documentation

Students may also satisfy the requirement by providing:

  • Laboratory titers showing immunity to measles, mumps, and rubella
  • Military immunization records
  • DD Form 214 (for current or former U.S. military service members)

Exemptions

Students who cannot receive the MMR vaccine should review the accommodation information below.

Exception

Students born before January 1, 1957 are not required to provide MMR documentation.

Requirement 2: Tuberculosis (TB) Screening

All students must complete the Tuberculosis Screening Questionnaire.

If your responses indicate a potential exposure risk, Curry Health Center staff will contact you with testing options and next steps.

Complete the TB Screening Questionnaire in the Patient Portal under 'Forms.'

 

Vaccine Accommodations (Exemptions)

Students who are unable to meet immunization requirements due to religious beliefs or medical reasons may request an accommodation.

Religious Accommodation

Complete and submit the following form to Curry Health Center:

Religious Exemption Form (HES 113)

Please submit your request well before registration to avoid delays or registration holds.

Medical Accommodation

Students requesting a medical accommodation must provide documentation from a qualified healthcare provider.

The documentation must:

  • Verify that one or more required immunizations are not considered safe for the student
  • Describe the medical condition or circumstance preventing immunization
  • Include the expected duration of the condition, if known

The statement must be signed by a healthcare professional who:

  • Is authorized to provide healthcare under state or Canadian law
  • Is authorized to administer the immunization(s) involved
  • Has previously provided care to the student or administered an immunization that resulted in an adverse reaction

Submit Medical Documentation

Please deliver the required documentation to the Medical Reception Desk at Curry Health Center.

Curry Health Center is located on the Northwest corner of Mountain Campus (634 Eddy Ave) for easy access for students.

The SARC and Wellbeing departments are found in the East entrance, closest to the School of Law building.

The Medical, Pharmacy, and Dental departments are found in the Center entrance, up the stairs. This is an accessible entrance.

The Counseling Services department is found in the West entrance, closest to the parking lot, and is downstairs. This is an accessible entrance.
